This information may be requested for the purpose of continuing care, billing, and patient health. The disclosed information may include all or part of your medical record. We will not release any information outside of those purposes and appropriate requests. Your privacy and trust is of the utmost importance to us.

For the general use of this site we do collect information. We collect information from you when you register on our site, download e-books or subscribe to our newsletter. When registering on our site, you may be asked to enter your name, e-mail address and phone number.
